Indication For Multiple Myeloma: Pomalidomide, in combination with dexamethasone, is indicated for adult patients with multiple myeloma (MM) who have received at least two prior therapies including a proteasome inhibitor and also lenalidomide and have demonstrated disease progression on or within two months of completion of the previous therapy.
Indication For Kaposi Sarcoma: Pomalidomide is indicated for the treatment of:
- Adult patients who carry AIDS-related Kaposi sarcoma after the failure of HEART therapy which stands for highly active antiretroviral therapy.
- KS (Kaposi sarcoma) in adult patients who are HIV-negative.
This indication is approved under the accelerated approval based on the overall response rate.
Dosage and Administration: Womens of reproductive potential should have negative pregnancy testing and contraception methods should be recommended in prior to start taking pomalid.
Recommended Dosage for Multiple Myeloma: The usual recommended dose is pomalidomide 4 mg once daily. Should be consumed orally with or without meals on Days 1 through 21 of every 28-day cycle until the disease progression. This drug should be given in combination with the dexamethasone.
Recommended Dosage for Kaposi Sarcoma: The usual recommended dose is pomalidomide 5 mg once daily. It should be consumed orally with or without meals on the Days 1 through 21 of every 28-day cycle until disease progression or unacceptable toxicity. Continue highly active antiretroviral therapy as HIV treatment in patients who are with AIDS-related KS (Kaposi sarcoma).
Administration: Swallow capsules whole with water. Never chew, break, or open the capsules. Pomalidomide 2 mg may be taken with or without food.
Contraindications: Pomalidomide is contraindicated in females who are pregnant. This Multiple myeloma drug can cause fetal harm when administered to a pregnant female . Pomalidomide is a specific thalidomide analogue. If the respective patient becomes pregnant while consuming this medication, the patient must be apprised of the potential and probable risk to a fetus.
Pomalidomide for multiple myeloma is contraindicated in patients who have demonstrated severe hypersensitivity for instance angioedema, anaphylaxis to it or any of its excipients.
Drugs That May Affect Pomalidomide Plasma Concentrations: In a very healthy subjects, co-administration of fluvoxamine, a strong CYP1A2 inhibitor, increased Cmax and AUC of the pomalidomide by about 24% and 125% respectively. Increased pomalidomide exposure could enhance the risk of exposure related toxicities. Co-administration of strong CYP1A2 inhibitors need to avoid for instance fluvoxamine and ciprofloxacin. If co-administration is unavoidable, dose of pomalidomide should be reduced.
Cigarette smoking may reduce the pomalidomide AUC due to the CYP1A2 induction. Patients are instructed that smoking can reduce the efficacy of the pomalidomide.
Overdose: Use of Hemodialysis is able to remove pomalidomide from circulation.
If you forget to take it: If it is not more than 12 hours prior to your upcoming dose, skip the dose you have forgotten and consume your very next dose when you are meant to. Otherwise, consume it as soon as you can, and then you should stick with the normal schedule of taking your medicine as before. Do not consume back to back dose in order to catch up for the specific dose that you have missed to take. If you are lacking confidence about what to do, discuss with your healthcare professionals. Storage and Handling: Should be stored at 20°C-25°C (68°F-77°F); excursions permitted to 15°C-30°C (59°F-86°F). Care should be exercised in the handling of this medicine. Never open or crush pomalidomide capsules. If powder from this drug contacts the skin, wash the skin immediately and thoroughly with soap and water. If this medication contacts the mucous membranes, flush thoroughly with water. Procedures should be followed for proper handling and disposal of hazardous drugs.
